    try this link

I called them before and they scheduled a demo but never found the time to sit down with them. From my brief encounter, they sounded quite professional. If ever you get the chance to try them out, do tell me how it goes.

or you can try filemaker. you can edit and write your own field name, however its quite complicated especially for newbies.

I'm with alvinrayyu on this one. I'm a very satisfied FileMaker Pro user and I think Bento would suit your needs. :D You can download the trial version first if you want. I posted a link to the demo video in the Bento thread in this forum.

Bento is a simpler (and cheaper!) version of FileMaker Pro.

Thank you to all your replies. Checked out Bento and requires Leopard OS. Are there any for Tiger?

I also use Filemaker Pro (since pre-migration from windows pa). I sync it to my treo using filemaker mobile and missing sync.

Can Bento somehow be linked to the iphone? I can't get rid of my treo because all my data are synced there.

You can try FileMaker Pro :) It's on the expensive side but you won't be disappointed.

Try OpenClinic or OpenEMR. Both are written on PHP and once installed you can access it locally via a web browser.
